The 2023 MDS updates include a focus on discharge planning. The Centers for Medicare and Medicaid Services (CMS) enhanced their focus on the importance of a seamless transition and coordination of care as our residents move between different healthcare settings. CMS has incorporated the discharge process in the MDS, rehospitalization measures, reimbursement, and the standard of participation.
