Course Description:

The 2023 MDS updates include a focus on discharge planning. The Centers for Medicare and Medicaid Services (CMS) enhanced their focus on the importance of a seamless transition and coordination of care as our residents move between different healthcare settings. CMS has incorporated the discharge process in the MDS, rehospitalization measures, reimbursement, and the standard of participation. Plan to attend this session and review the importance of a successful transition of care. All attendees will receive a useful tool related to discharge planning.

Objectives

Upon completion of this presentation, participants should be able to:

1. Review the changes in the MDS effective October 2023 as well as the RAI process encompassing comprehensive discharge planning.
2. Discuss the significance of a comprehensive discharge plan of care and organizational outcomes including changes to quality measures.
3. Understand the application of INTERACT in improving the resident discharge process.
